Patient: 44 year old female with long standing L4 spondylogenic spondylolithesis Grade II and progressive secondary osteoarthritic and stenotic changes at the L4 and L5 levels.
Presentation: Acute left sciatic pain with foot drop and associated 75% weakness in foot flexors after an 18 hour drive.
SRA Evaluation: T6 left axial spinal reflex mechanism with reflexive interspinous and iliolumbar ligament contractures accentuating progressive spinal instability and compressive forces at the L4/L5/S1 disc levels.
Treatment: CMT 2-3x per week for 8 weeks with specific clinical and home physical therapy strengthening of the intrinsic paraspinal musculature in extension and rotation per SRA protocols over 12 weeks. Outcome was 95% return of flexor strength without sciatica and resumption of horseback riding activities without aggravation of chief complaint.
Recommendations: Maintain intrinsic muscle strength through home therapeutic exercise and pursue CMT on a PRN basis (currently 1x per 6-12 weeks).
